It is far easier to prevent bone mass than to return bone mass to normal once osteoporosis is fully developed. All investigators agree that there is a period of accelerated bone loss during the years immediately following menopause and that it would be desirable to intervene at this point where it were possible to target therapy to those women at greatest risk. Christiansen has devised a simple means for identifying women who subsequently will have the greatest postmenopausal bone loss and has evaluated various forms of prophylactic therapy. These studies will be reviewed and extended. Therapeutic agents for established osteoporosis fall into two general classes - those that decrease bone resorption and those that increase bone formation. Antiresorptive agents prevent or slow further bone loss but do not increase bone mass substantially. Of the available antiresorptive drugs, Lindsay will review treatment with estrogen, progestins, and calcium and Chesnut will review treatment with calcitonin, anabolic steroids, and diphosphonates. Only agents that increase bone formation to a level greater than bone resorption are capable of substantially increasing bone mass. Four such programs will be reviewed by Meunier - sodium fluoride, PTH, the combined calcitonin and phosphate regimen, and the ADFR regimen. With increasing understanding of factors regulating bone turnover, we can expect even better regimens programs to be developed in the future.
